Keep reading to learn more!

Contact Form and Application

The application process to become an egg sharer begins when you complete our simple contact form, which can be found here. Fertility United will then email you a full application to fill out.

The full application will allow us to learn a bit more about you so that we can determine if you are a good candidate for our program and if so, help us find you a perfect match to share your eggs with. Within this form, you will see questions about reproductive history, educational history, family/genetic background, and general health history.

Of course, we also want to know about your personal achievements, interests, and goals. Our recipients want to help you achieve your vision by sponsoring your egg freezing, so don’t skimp on the details! This is a chance for you to let your personality, passions, and dreams shine!

In-person Q & A

Within a week of submitting your full application, we will email you copies of paperwork to review in addition to setting up a face-to-face meeting where we will answer any questions you may have about egg freezing, egg sharing, anonymity, medications, injections, possible risks, as well as a thorough review of the paperwork you’ve received.

If you live locally in the Los Angeles area, an egg sharing coordinator will meet you in person. A web conference meeting will be arranged for those who live out of the area. This is your chance to ask us absolutely anything you may be wondering. There are no bad or stupid questions. We want you to be clear about the procedures involved and feel confident moving forward.

Consents signed and final acceptance into the program

After the interview has been completed, it will be your job to return signed paperwork (if your interview was by web conference) and other required documentation.

Some examples of documents we require are:

  • Identity verification documents (driver’s license or ID)

  • Natural photos at various ages

  • Education verification documents (if applicable)

  • Egg Sharing informed consents

Once documents are received, we will email you confirmation of acceptance into the Fertility United Egg Sharing Program!

The Next Step

After final acceptance into the program, we will begin the next phase of the journey, matching you with a family hoping to grow theirs. The application process can range in length from anywhere to a week to a month depending on your interview availability and the speed in which consents and necessary documents are received in our office.

The application process can seem intricate, however, it is set up that way for the purposes of informing you about the process to come and prepare you for all the possibilities associated with egg sharing; ensuring that you are making the best choice for your life.

Happy sharing!
